*Position Summary*
The WAIT House Health Home Care Manager, contracted with Lead Health Home Adirondack Health Institute (AHI), will visit members in their home, in the community, and/or clinical settings as preferred by the member, assessing and collaboratively planning interventions to help members manage social, emotional, medical, economic, and environmental concerns.
This role will assist members with identifying needs, priorities and obtaining community assistance through referrals and coordination with appropriate local and regional resources.
The Care Manager will maintain an average caseload of 20+ members based on a member’s level of acuity.
The Care Manager is responsible for, developing ongoing relationships with Health Home candidates and enrolled members to engage these members in the service delivery process, documenting all care activities and known process outcomes for each member on the caseload.
The Care Manager will also develop and maintain collaborative working relationships with community-based organizations, social service agencies, and other service providers to better facilitate service coordination for enrolled members.
*Qualifications:*
* Bachelor’s degree required and one years’ experience in a community residence or related human service field.
* Demonstrated commitment to providing direct services to persons with serious mental illness, alcohol or substance use.
* Valid driver’s license and provision of evidence of appropriate insurance coverage.
* Effective crisis intervention skills.
* Effective verbal and written communication skills.
* Excellent organizational skills, flexibility, and initiative.
* Must possess the ability to work autonomously and have a strong knowledge of community resources (medical, behavioral, social).
*Responsibilities:*
* Support organizational commitment to regional care management needs and quality while abiding by all time regulations and deadlines enforced by The Care Management Agency and Lead Health Home.
* Offer and provide care coordination services to both outreach and enrolled members
* Develop collaborative person-centered care plans and maintain thorough documentation of efforts to assist members in achievement of care plan goals
* Participate in collaborative case review, discharge planning, and other interdisciplinary and multidisciplinary meetings
* Coordinate with managed care organizations (health insurance companies) to improve outreach and coordination of care efforts
* Maintain compliance of documentation as per the Care Management Agency and Lead Health Home policies and procedures
* Communicate resource needs to leadership to improve quality of care and to promote value-based outcomes
* Complete all trainings required as per the Care Management Agency and Lead Health Home
